Deep buttock pain that becomes worse after sitting. Pain when walking uphill, climbing stairs, running or turning in bed. Aching, burning or tingling that may travel into the back of the thigh. A feeling that one hip or leg is tight, weak or unreliable. Temporary relief from stretching, massage or foam rolling, followed by the same symptoms returning. Previous treatment has focused on the piriformis or lower back, but progress has stalled.
These symptoms are often labelled piriformis syndrome, but the label does not explain why the area remains sensitive. The piriformis may be contributing, protecting another part of the system, or sitting alongside neural sensitivity or referred pain. Your assessment needs to establish which findings are relevant in your individual case.
Pain in the buttock or back of the thigh can come from several overlapping sources. These include local piriformis or gluteal sensitivity, the lower back, the sacroiliac region, hip structures, hamstring attachment, or irritation involving the sciatic nerve. A scan or symptom label alone may not show which factor is driving what you feel today.
We compare your history with movement, strength, sensation, reflexes and symptom behaviour. This helps distinguish a modifiable functional pattern from signs that need medical investigation. Progressive weakness, loss of bladder or bowel control, numbness around the saddle area, unexplained illness or severe symptoms after trauma require urgent medical assessment.
Stretching, massage and foam rolling can be helpful when they match what the area needs. If relief is brief or symptoms repeatedly worsen, however, the piriformis may be responding protectively rather than simply needing more stretching.
Functional neurology and P-DTR take a brain-based test-and-retest approach. We assess relevant muscles, joint receptors, sensory information and movement patterns, then apply an appropriate input and immediately retest. This ‘peeling the onion’ process helps identify which layer is currently holding the system back. Responses vary, and no single finding is assumed to explain every case.
Your first appointment is a £100, 60-minute Diagnostic First Session. It includes a detailed history, neurological and movement assessment, treatment where appropriate and immediate retesting to show whether a finding is relevant.
You will receive practical brain-based exercises where appropriate, a written report within 24 hours and an included 30–40 minute Zoom findings call. The purpose is to give you a clearer explanation and realistic next steps, without pressure to book further care. Some people experience meaningful functional change in the first session; the size and duration of the response vary.
This approach may be relevant if deep glute or leg symptoms keep returning; sitting, walking, running or stairs remain limited; stretching only helps temporarily; previous physiotherapy, chiropractic care, massage or injections have not produced lasting progress; or the diagnosis does not fully explain why symptoms fluctuate.
“Following treatment with Sam, the things I was doing to help actually started to work. Stretching no longer aggravated pain.”, Michael Holland
“As movement improved through my ankles, knees and hips, my back pain reduced significantly.”, Paul Hayes
“The results were evident within a short period of 5 sessions. My pain level, range of motion, and running ability significantly improved.”, Matthew Howell
